Case Managers

o   Homeless Community Partners Case Managers (HCPCM)

o   Mission: A format for case managers and case workers to assist the homeless populations in Salt Lake City and the surrounding areas to meet and share information about community resources.

o   Background- Inspired by focus groups conducted by the Department of Workforce Services Central Regions’ One Stop Task Force and incorporated by the Salt Lake Homeless Coordinating Council, since 2007 these meetings have been held regularly, coordinated by volunteers and hosted by various agencies.  Meetings highlight agency services, involve peer to peer networking, offer Case Manager training and share community resource information.  All Case Managers that assist the homeless populations are welcome to attend.  To get on the contact list for upcoming meetings, please contact one of the coordinators.
