Utah Balance of State Continuum of Care is made up of seven regions. They are: Bear River Associates of Government (AOG) Counties: Cache, Rich and Box Elder City: Logan Uintah Associates of Government (AOG) Counties: Duchesne, Daggett and Uintah Southeastern Associates of Government (AOG) Counties: Carbon, Emery Grand and San Juan Six County Associates of Government (AOG) Counties: Juab, Sevier, Piute, Millard, Sanpete, and Wayne Five County Associates of Government (AOG) Counties: Beaver, Iron, Garfield, Kane and Washington City: Saint George Davis County: County: Davis Cities: Clearfield and Layton Weber County: Counties: Weber and Morgan City: Ogden.
